A Florida construction company was issued willful citations alleging failure to provide cave-in protection to employees working inside an excavation that contained unstable soil, and allowing excess soil to be placed too close to the edge of a trench wall. The company was also issued serious citations alleging failure to adequately train employees, exposing workers to hazards from ductile iron pipe being lifted overhead and allowing employees to use a ladder of insufficient length on an unstable surface.
