Two workers fell 47 stories when their scaffold became detached from a window washing rig attached to the building’s roof. Miraculously, one man was critically injured, but survived. The other worker died. OSHA cited City Wide Window Cleaning for serious violations including the lack of fall protection, training and inspections, and the lack of a vertical lifeline.
