OSHA opened an investigation following the death of an employee in January and later expanded it to cover the entire facility. The company was cited for willful failure to protect all open-sided floors and work platforms from potential fall hazards. OSHA also cited 16 alleged serious violations including machine guarding and energy lockout hazards and training, and health monitoring deficiencies. Two repeat citations were issued for failure to lock out hydraulic and electrical energy where necessary. Proposed penalties in this case total $163,000.
