It's not unusual for workplace safety inspectors to spot workers working at heights without fall protection, but the location of this unsafe worksite is somewhat ironic, in that it's a crematorium in Wales. A maintenance company was fined after two workers were observed working on the flat roof without any fall protection. The ladder they were using to access the roof wasn't properly secured and this situation endangered not only the workers, but also members of the public walking below.
