OSHA has issued citations for numerous alleged hazards relating to inadequate safeguards involving formaldehyde and various chemical, mechanical and electrical hazards. OSHA says the plant lacked a process safety management program and procedures to proactively assess hazards associated with processes and equipment using large amounts of formaldehyde.
