OSHA opened an inspection after a worker lost two fingers while operating an unguarded saw. The citations allege a cross-section of mechanical, chemical, electrical, health and fire hazards including unguarded saws, deficiencies in required safeguards for employees who cleaned up blood after the incident, a lack of PPE, a lack of forklift operator training and unlabeled containers of hazardous chemicals.
