Un empleado varón de 52 años de edad falleció como consecuencia de la exposición a una atmósfera peligrosa de cloro gaseoso. Una manguera de transferencia de productos químicos para hipoclorito sódico (lejía) se conectó accidentalmente a la línea de llenado de ácido hidrofluorosilícico; cuando los productos químicos se combinaron se creó un gas cloro.
