Must Employers Maintain Records of Accidents or Injuries?  Yes Must Employers Report Injuries? Yes When Must Report be made? Within 6 days of receiving notice of employee’s claim. For fatal or catastrophic injuries, within 8 hours after the accident is reported to any agent or employee of the employer. Penalties for Failure to Report $1,000 for each violation.
