Must Employers Maintain Records of Accidents or Injuries? 

Yes

Must Employers Report Injuries?

Yes, all injuries causing an absence of a day or more or requiring medical attention.

When Must Report be made?

Within 72 hours, Sundays and holidays excluded, after the injury.

Penalties for Failure to Report

$100 per violation. Failure to file annual statistical report upon request of commissioner of labor and industry may result in $1,000 fine. Willful false statements or representation in order to receive lower workers’compensation premium may result in penalty of not more than $5,000

Carriers/employers are required to establish and report to the Department and the claimant in each claim the day of the week on which payment shall be mailed or deposited.  This is true whether benefits have been awarded or are not in dispute.
