Must Employers Maintain Records of Accidents or Injuries?

No

Must Employers Report Injuries?

Yes, injuries by accident and occupational diseases arising out of and in the course of employment that cause either disablement or death of the employee.

When Must Report be made?

Report must be made as soon as possible, but not later than 14 days after the accident, where there is a permanent disability or the employee does not return to work within 7 days. Where there is no permanent impairment and the employee returns in less than 7 days, report must be made by the 15th day of the following month.

Penalties for Failure to Report

$25 for each 15 days past the required date for filing.
