Must Employers Maintain Records of Accidents or Injuries? 

Yes

Must Employers Report Injuries?

Yes, all injuries resulting in 7 or more days lost from work.

When Must Report be made?

Immediately on employer’s knowledge of the accident. Second report should be made 3 weeks after the accident.

Penalties for Failure to Report

Not less than $10 nor more than $50 per violation.

Penalties for Failure to Carry Insurance

Penalties up to $5,000, additional penalty of up to $5,000 for period of more than 10 days. If determined employer knowingly or deliberately provide protection, a stop-work order requiring the cessation of all business operations of that employer at every site at which the violation occurred may be issued.
