Must Employers Report Injuries?

When Must Report be made?
As soon as possible, but no later than 5 days after employer learns of the occurrence of the injury. If the injury results in a disability of 3 days or more, employer shall file a supplemental report as soon as possible, but no more than 7 days after the injury.
Penalties for Failure to Report
Civil penalty of up to $2,500 for failure to file a first report.
