Must Employers Maintain Records of Accidents or Injuries? 

Yes

Must Employers Report Injuries?

Yes, all accidents resulting in injury or death.

When Must Report be made?

In the case of serious injury or death, report by telephone must be made within 48 hours and a written report must be filed within 7 days. For incapacitating injuries resulting in inability to work for 3 calendar days or more, report shall be made within 10 days. Selfinsured employer must report the injury no later than 14 days after its occurrence, If an injury is reported and death subsequently occurs, employer shall report within 48 hours of receiving notice, followed by a written report within 7 days.

Penalties for Failure to Report

Up to $500 for each failure to report.
