Must Employers Maintain Records of Accidents or Injuries?
Yes
Must Employers Report Injuries?
Yes, all injuries involving medical treatment, transfer to another position, loss of consciousness, restriction of movement, or the loss of more than 3 scheduled workdays. An employer need not report minor injuries requiring no more medical attention than on-site first-aid treatment.
When Must Report be made?
2 working days after an accidental death, or between the 15th and 25th of each month, unless required sooner by rule of the industrial commission.
Penalties for Failure to Report Petty offense.
