Must Employee Give Notice of Injury or Illness to Employer? 

Yes

Must Notice Be in Writing?

No

When Must Notice Be Given?

Within 14 days of the injury.

Exemptions to Notice Requirements

If employee can show that he failure to give notice was the result of mistake, inadvertence, ignorance of fact or law, or inability, or was due to the fraud, misrepresentation, or deceit of another person, or any other reasonable cause or excuse, and that employer was not prejudiced by the lack of notice.

Effect of Failure to Notify Employer 

Compensation may be barred. 

Other Provisions

Regardless of other provisions, if employer has not received notice within 90 days, compensation will be barred.
