Must Employee Give Notice of Injury or Illness to Employer? 

  Yes 

Must Notice Be in Writing? 

  No 

When Must Notice Be Given? 

  Within 30 days of the injury; within 3 months in the event of death.. 

Exemptions to Notice Requirements 

If employer has knowledge of the injury, if the employee is unable to give notice by reason of mental or physical incapacity, or if employee fails to give notice because of a mistake of fact.   

Effect of Failure to Notify Employer 

  Claim for compensation is barred.
