Must Employee Give Notice of Injury or Illness to Employer? 

Yes

Must Notice Be in Writing?

Yes

When Must Notice Be Given?

Within 30 days of  injury or death, or 30 days of when employee or beneficiary becomes aware of the relationship between the employment and the injury or death.

Exemptions to Notice Requirements

If employer had knowledge of the injury or death, or the relationship between the employment and the injury or death. The mayor may excuse the failure to give notice if employee was unable to give notice.

Effect of Failure to Notify Employer 

May bar compensation.
