Twenty-five Serious Safety Violations Cited at CITGO Asphalt Refining Co. Plant

A refinery in Georgia is facing $110,000 in proposed penalties for 25 alleged serious safety violations. Allegations include failure to take adequate precautions when working around combustible liquids, failure to inspect and test process equipment in accordance with good engineering practices, inadequate emergency response plans, inaccurate pipe and instrumentation diagrams and failure to inform contractors of known hazards.
