Allegations of One Willful, 31 Serious Violations Bring $192,350 in Proposed Fines

The willful violation alleges failure to enforce lockout/tagout procedures to prevent accidental start-up of machinery. The 31 serious violations allege inadequacies in training workers; deficiencies in clothing and equipment needed for fighting fires; inadequate and missing machine guards, inadequate labeling of chemicals and electrical deficiencies.
